Alliance Community Hospital is an independent 104-bed hospital based in Ohio with an attached 78-bed skilled nursing facility. They don’t have any risk-based contracts. Being one of the largest employers in the community, the hospital provides self-funding health insurance benefits for its employees and their dependents. Some key steps in care delivery model towards clinical transformation
Health Coach: Alliance Community Hospital initiated a health coaching program last year with local university students.
The objective was
Ø To avoid emergency admissions
Ø To reduce Hospital readmission
The students were part of a team that includes a physician, medical director, program director, a nurse coordinator, a pharmacist, a dietician, a social worker/counselor, and a hospital chaplain. Each university students was assigned to a patient who is suffering from a chronic care condition and other challenges like low income, illiteracy etc. The goal was to understand the barriers in keeping the patient in good health and overcoming the challenges.
Access to Care: Alliance Community hospital opened an on-site clinic, which operates on two-half days a week. It consists of a community employer’s office that provides better access to care after noticing increased ED visits of the employees. This helped the employees to receive primary care service at ease. Recently the hospital started operations after-hours and weekends clinics to further support employees, their dependents and others in the community, in order to minimize the ED admissions.
Data into information: Alliance community Hospital partnered with VitreosHealth- A Predictive & Prescriptive Risk Analytics Company, to analyze insurance claims of its employees and their dependents. The analysis showed that about 20% of its employees & their dependents are at high risk/ high cost, causing $1,179/month in their healthcare costs. While 35% of its workers are at high-risk/low-cost, resulting for $58/month of their healthcare spending.
More interestingly the analysis found that about 40% of the employees and their dependents fall into “Hidden Opportunity” category. Though healthcare costs of those employees or their dependents were low, each of them had a diagnosis code for their chronic condition but there are gaps in the care.
The review of 3 years claims data helped the hospital administrators to understand why the category is called “Hidden Opportunity” and how many employees and their dependents are moving from high-risk/low-utilizers to high-risk/high-utilizers category during this time period.
VitreosHealth’s analysis on claims data also showed that Alliance Community Hospital could save more than $850,000 or 12% of their total healthcare spending, if
