Care Management Technologies Announces New Client: Association of Community Mental Health Centers of Kansas

Seven of the 27 ACMHCK members serving as Medicaid Behavioral Health Homes will use CMT’s ProAct to demonstrate effectiveness in improving quality of care and lowering costs for consumer’s enrolled in their health home model.

ACMHCK’s CMT 7 will utilize ProAct’s ability to combine clinical and claims data to gain a clear, 360° view of consumer care and needs. ACMHCK will also take advantage of ProAct’s more advanced features – analyzing consumer data against an anonymized claims database with more than 800 million Medicaid, Medicare and private insurance claims data points; tracking comparative performance improvements based on customized and industry metrics; providing action based analytics through interoperable platforms at the agency level and utilizing CMT’s risk alerts to prevent emergency room and hospital readmissions. In addition, the CMT 7 will benefit from groundbreaking work by CMT’s clinical and health experts, which ensures that the latest evidence-based behavioral health research outcomes are incorporated directly into the analysis engine and ready for application and interventions at the consumer specific level.
Using ProAct, the ACMHCK CMT 7 will be able to organize and analyze all available data in order to better understand and manage risk, taking into account everything from adherence markers and gaps in care to costly co-morbid conditions and underlying addiction or substance dependency concerns. Drawing on more than 200 proprietary and validated clinical rules, the CMT 7 will use ProAct to guide improvements in decision-making that foster more efficient and effective delivery and management of care.
