CSS Health Announces Launch of CCD Integration
CSS Health has partnered with the Rio Grande Valley HIE to launch new CCD integration in a leap towards interoperability
BUFFALO, N.Y., October 6, 2017 (Newswire.com) - Clinical Support Services, Inc., a leading provider of Medication Therapy Management solutions, is excited to announce the launch of their Continuity of Care Document (CCD) Integration.
CCD Integration moves CSS and MTMPathTM significantly along the interoperability path. With the new CCD Integration functionality, clinicians using the MTMPathTM platform can now integrate with any physician or hospital electronic record system to consume CCD documents and effortlessly build a patient profile for medication management review.
CCD integration:
- Allows for robust therapy gap identification
- Reduces the administrative burden of data entry and maximizes valuable pharmacist time
- Allows clinic-based pharmacists to significantly scale their medication management efforts over large populations of patients
To launch its CCD Integration capability, CSS has partnered with the Rio Grande Valley HIE. Rio Grande Valley HIE aggregates CCDs from its partner clinic organizations. Clinicians located at partner facilities can request a medication review by a clinic pharmacist using the MTMPathTM platform by submitting a CCD transmission request. The CCD request routes a CCD from Rio Grande Valley HIE to MTMPathTM where it is consumed and a patient profile is prepared with demographic, medication, diagnosis and lab data for medication management review.
