The survey provided by the human resources training company questioned 1,358 participants about their employer's performance appraisals, of which 94% reported that their organization conducted these evaluations. Of this 94%, 77% rated their company's performance evaluation methods with a score of C or worse, meaning the vast majority of participants were unhappy with their employer's evaluation procedures.

The report lists the three worst mistakes that participants believed their employers made in evaluations:

1) Not following up with employees after the review
2) Placing the weight of all employee responses equally, regardless of hierarchy
3) Only focusing on the last performance
