Westbury, NY, January 12, 2016 (Newswire.com) - RXVIP Concierge today announced that it has signed a Partner Agreement with CareSync™ to help deliver Chronic Care Management (CCM) and VIP Pharmacy services directly to patients at the point-of-care.

This partnership represents an expansion of the RXVIP Concierge business model focused on enhancing the role of the Pharmacist in the heath care continuum.

This new partnership allows physicians looking to provide CCM services to quickly get started without sweeping changes to workflow or expensive new technology purchases. Personalized implementation, including Enrollment Specialists to help facilitate patient onboarding, is provided with zero out-of-pocket expense to the practice.  RXVIP Concierge Pharmacists and their CCM Care Team made up of PharmDs and PharmD Students will work in physicians’ offices to help enroll and manage patients at the direction of the physician and staff with no change in the workflow and no upfront cost to the practice.  The RXVIP Care Team will provide each practice with full-time staffing to work directly with the practice to help deliver better patient outcomes.
