Wellbe Debuts Guided CarePath for the Perioperative Surgical Home

Provider and patient facing tools specifically designed for step-by-step surgery guidance

Wellbe, Inc. announced today the availability of its Guided CarePath for the Perioperative Surgical Home (PSH).

Wellbe offers the only cloud-based platform that combines vital tools for patient engagement and care team coordination across the perioperative continuum. This patient-specific, coordinated pathway approach ensures higher patient engagement across the episode and results in reduced risks, optimal costs and a better patient experience.

This usually led to poor communication between providers and patients, and even poorer communication between the providers themselves. The Guided CarePath is accessed by patients via the web on PCs, tablets or smartphones. The comprehensive program covers onboarding, risk assessments and pre-surgery preparations, inpatient reinforcement, discharge/transitional care planning, and recovery support during the post-acute period.

Patients are enrolled when a treatment decision is made and, like a GPS, the guided program delivers everything the patient and provider needs as turn-by-turn directions at exact moments in the journey of care, including: shared decision-making, self-care instructions, assessment measures, education (including videos), preparation checklists, and feedback surveys.
